]1) I think the motor blasted because 1-st stage grain isolators (grain burn inhibiter) have closed nozzle than second stage having start. May be you need not isolated grains in 1-st stage ? Or bond grain isolators to liner or motor case in few points around upper end of each grain.

The nozzle was not blocked by the inhibitor, even though that was our
first theory. Examination of the pressure data showed that the casing
failed as the pressure was already starting to reduce, and there was no
pressure "spike". The casing failed because of a inferior welded seam
combined with reduced strength when hot.
]2) I think you should use aluminum alloy tube for second stage - it will cooling by air in real flight.
